stunnel で詳細ログを見て、どんな SMTP コマンドを発行・応答が
得られているか観測してみるとどうでしょうか。

Postfix 側の詳細ログを見るなら

  debug_peer_level=10
  debug_peer_list=127.0.0.1

などとするとか。
